The application name in your TOTP app is currently determine by the Title field in the global settings in IAM.

It is not possible to get an icon in your TOTP app at this moment, though it would make sense if the Application icon in the global settings would be applied. You could create an idea for this, or an alternative that you think would be more helpful.

Upon further inspection, it actually does not seem possible to include an icon in the QR code which the TOTP app can then show for that entry. Apparently, some TOTP apps allow you to set an icon yourself and some TOTP apps recognize common issuers (like Microsoft, Google, etc.) and automatically show a built-in icon for those.

Maybe it is possible to drop an associated icon for a domain somewhere, like what is possible for email accounts. 

For the rest it is not very very important, but it would have been nice.
